Observasi Penilaian Resiko Jatuh di Ruang Rawat Inap Raudhah 2 Rumah Sakit Umum Daerah dr. Zainoel Abidin Banda Aceh

Abstract
The risk of falls among hospitalized patients is a critical patient safety issue that must be prevented, especially in the pediatric post-surgical inpatient ward, where accurate assessment and management of fall risk by nurses play a crucial role in minimizing fall incidents that may lead to serious complications. This study aims to describe the implementation of fall risk assessment and management conducted by nurses in the Raudhah 2 Pediatric Post-Surgical Inpatient Ward at Dr. Zainoel Abidin General Hospital, Banda Aceh. The method used was descriptive observation with data collection carried out over three shifts (morning, afternoon, and night) for three consecutive days from March 10 to 12, 2025, using a questionnaire developed based on the ward’s Standard Operating Procedures (SOP). The results showed that the implementation of fall risk prevention measures and communication of risk levels varied, with respective execution rates of 55.5% and 66.7%. In conclusion, fall risk management in the Raudhah 2 Pediatric Post-Surgical Inpatient Ward has been conducted well, particularly in the placement of risk markers; however, there is a need to improve consistency in the execution of preventive actions and risk communication to optimize fall prevention efforts.
