Asuhan Keperawatan pada Pasien dengan Chronic Kidney Disease Stage V di Ruang Aqsha 3 Rumah Sakit Umum Daerah Zainoel Abidin Banda Aceh

Abstract

This study employed a case study design using a family nursing care approach, which includes the stages of assessment, nursing diagnosis, planning, implementation, and evaluation. The purpose of this scientific work is to provide holistic nursing care for a patient with Stage V Chronic Kidney Disease (CKD) in the Aqsha 3 Ward of dr. Zainoel Abidin General Hospital, Banda Aceh. The assessment results revealed that the patient experienced weakness, shortness of breath, anasarca and peripheral edema with grade II pitting edema, creatinine level of 3.20 mg/dL, urea 34 mg/dL, albumin 2.84 g/dL, nausea and vomiting, as well as hypoglycemia. The identified nursing problems included hypervolemia, ineffective airway clearance, activity intolerance, risk of nutritional deficit, and unstable blood glucose levels. The interventions carried out involved collaboration for hemodialysis therapy twice a week, renal diet education, oxygen therapy, monitoring of fluid intake and output, encouraging the use of the “sipping ice” technique, teaching effective coughing, performing postural drainage and chest physiotherapy, maintaining oral hygiene, and instructing the patient to gargle with salt water. The evaluation results showed improvement in most of the identified nursing problems within five days of care. It is expected that the family will continue to provide support to the patient to ensure adherence to treatment and promote optimal recovery.
